In which hospitals I can have a cashless admission
facility?
A list of network hospitals/nursing homes is provided, along where you can
avail cashless hospitalization. In case you need any further details of the
hospitals in any city, you can contact them on the numbers given in the list
of hospitals or contact nearest branch. Constantly new hospitals are
added in network across the country in order to provide better services and
some may be put under sanctions.
You can visit www.mdindia.com for an updated list of hospitals. We request
you to understand all the terms and conditions of the policy in detail,
especially exclusions and illness/diseases covered under the policy.

Check list to follow during hospitalization.
1. Inform the Medical Officer of Symbiosis Centre of Health Care.
2. Approach to insurance department of hospital and get ‘MD India RAL’ form.
3. Fill in the details as required in consultation with the officials of
Hospital.
4. Submit the form to Hospital along with photo copy of ID-card /Insurance
card.
5. To ensure the Officials at Insurance Dept.of Hospital sends the required
document at the earliest to MD India for authorization.
6. In case of Road /Rail Traffic Accident, MLC or Panchanama report by
Police Dept. is also needed.
